Building Healthier Communities: NCCDPHP Partners with Communities to Advance Health Equity

CDC’s National Center for Chronic Disease Prevention and Health Promotion (NCCDPHP) is taking innovative approaches to building healthier communities. As NCCDPHP Director Dr. Karen Hacker explains, our ability to be healthily can be affected by social determinants such as economic status, education, and access to health care. To make sure that everyone can be healthy regardless of who they are, NCCDPHP is partnering with a wide variety of communities and organizations to equitably address social determinants of health and advance health equity.
